Goals
The overall goal of the proposed study is to
document the effectiveness of a program to increase the identification of
depression among substance abusers and to improve the treatment services
offered to persons with these complex co-occurring conditions. This goal
will be addressed by conducting in-person psychiatric assessments on 500
consecutive adult admissions to the Chestnut Health Systems (CHS)
substance abuse treatment programs in Madison County, Illinois. Subjects
with current DSM-IV major depression (n=120) will then be randomized to
the enhanced "psychiatric case management" intervention group (n=60) or to
a no case management, "treatment as usual" group (n=60). Each patient in
the case management group will be assigned to a clinical case manager who
will educate him/her about depression and will provide individualized case
management services designed to facilitate referral to and compliance with
psychiatric care. All 120 subjects with depression at baseline will be
followed up at 6 and 12 months to assess short and long term outcomes,
using CHS records, State database records, in person interviews, and urine
drug testing. The specific aims of the study are to:
Evaluate whether providing information from standardized psychiatric
assessments to the case managers improves the detection of psychiatric
disorders compared to treatment as usual;
Determine the short term (6 month) and long term (12 month) impact of
providing or not providing additional psychiatric case management services
to substance abusers with depression, in terms of:
a)
Substance-related and non-substance-related psychiatric diagnoses and
treatment services (from records);
b) Subsequent substance use and psychiatric symptom severity (from
interviews and urine drug testing), and
c) Consumer satisfaction with treatment (from interviews); and
Cooperate with CSAT
on evaluation of cost and benefit cost studies of the intervention.
Taking advantage of the wealth of data collected at baseline on 500
subjects, the proposed work also includes components for secondary analyses.
These analyses will address methodological issues influencing the assessment
of comorbid psychiatric disorders in order to determine the sensitivity,
specificity, positive predictive value and negative predictive value of
different thresholds on the initial screening instruments. The purpose of
these secondary analyses will be to determine when additional psychiatric
interviewing is most useful. In particular, knowing how the Global Appraisal
of Individual Needs (GAIN), which is required for this project, maps onto
more detailed diagnostic assessments will help to inform this study and
perhaps all of the studies in this group funded under the CSAT Co-Occurring
Disorders Study program.
